Registered Nurse

​

Qualifications:

  • Must have graduated with an associate degree in nursing.

  • Must have at least passed the U.S. NCLEX-RN exam. 

  • Must have 6 months experience working as an RN for a hospital or nursing facility. The company will provide on-the-job training in home health/hospice area.

  • Self-directed and able to work with minimal supervision. 

  • Prolonged or considerable walking and standing. Able to lift, position and/ or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform nursing skills. 

  • Possess current driver’s license and automobile insurance.

Responsibilities:

  • Conducts and completes initial, comprehensive and update of the patient’s assessment.

  • Provides hospice services requiring nursing skills in accordance with the Plan of Care.

  • Assist patient and family in developing goals and areas of need, assist in developing treatment plans which are assessed regularly.

  • Assess patient’s status, develop, implement and evaluate individualized Plan of Care in collaboration with the Interdisciplinary Group to meet the needs and maintain continuity of care.

  • Monitor, assess and record client progress against Plan of Care and make adjustments accordingly.

  • Completes all clinical notes in a timely manner in accordance with the Hospice’s policy.

  • Coordinates patient care with the members of Interdisciplinary Group.

  • Participates with QAPI and Infection Control Process.

  • Timely submission of clinical record audits to ensure completeness and accuracy of clinical records.

  • Provides patient and family education

  • Provides and maintains a safe environment for the patient.

  • Assist patient and family/ Primary Care and other team members in providing continuity of care.

  • Provides professional management of patient’s care in all settings: (home, SNF, RCFE and B&C)

  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s conditions and care given. 

  • Communicates with the physician regarding the patient’s needs and reports changes in patient’s condition in a timely manner. Obtains / receives Physician’s orders as required.

  • Communicate with community health related persons to coordinate Plan of Care.

  • Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and family/Primary Care Givers.
