On-call Nurse
Qualifications:

Must have graduated with an associate degree in nursing.

Must have at least passed the U.S. NCLEX-RN exam.

Current BLS Certification.

Must have 6 months experience working as an RN for a hospital or nursing facility. The company will provide on-the-job training in home health/hospice area.

Self-directed and able to work with minimal supervision.

Prolonged or considerable walking and standing. Able to lift, position and/ or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform nursing skills.

Possess current driver’s license and automobile insurance.
Responsibilities
-
Takes calls and responds to calls within 15 minutes.

Makes necessary visits after hours, weekends, holidays per patient's needs if on-call runners are not available.

Provide written documentation on Nursing progress/ Visit notes to the office by 08:30 am the following morning or if a weekend, Monday morning.

Report’s patient’s conditions to RN/Case Manager or DPCS to be documented in IDG Notes and to be discussed in IDG meetings.

Maintain an “On-Call log” listing each call received. Information will include the following: Time of call, patient’s name, name of caller, nature of call, action taken.

Completes and submits documentation in accurate, legible and timely manner in accordance with the policy.

Administers prescribed medications and treatments as permitted by scope of practice.

Provides Hospice services requiring nursing skills in accordance with plan of care.

Educates and instructs patients and Families.

Completes, maintains and submits accurate and relevant clinical notes regarding patient’s conditions and care given. Records pain / symptom management / outcomes as appropriate.

Communicates with the physician regarding the patient’s needs and reports changes in patient’s condition in a timely manner. Obtains / receives Physician’s orders as required.

Communicate with community health related persons to coordinate Plan of Care.

Teaches the patient and family/ Primary Caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and family/Primary Care Givers. Works in concert with the Interdisciplinary Group.
